At Elim Care, we continue to focus on supporting our employees’ health and well being. With that goal in mind, Elim has partnered with US Wellness to bring you health screenings (Blood Pressure, Body Mass Index, Total Cholesterol, and Glucose) to determine your risk of cardiovascular disease, among other health risks.
Onsite Health Screening FAQ’s
What is a health screening?
The onsite health screening is one way to assess your risk for cardiovascular disease, diabetes, and high blood pressure. It does not replace your annual physical exam.
A health professional will collect a drop of blood from your finger to measure your total cholesterol and glucose. Your health screening results will be available immediately. The US Wellness onsite nurse will review your results and share health and wellness resources that are available to you. Individual information is confidential. Elim will receive a group report that summarizes the results of the health screening events, but will not receive your personal information.
How much time is required to participate in the health screenings?
Each screening takes approximately 10 to 15 minutes from start to finish.
Who is eligible to participate in the onsite health screenings?
Employees who are currently Medica members may participate in the onsite health screening. You will need to sign a consent form prior to your screening. Employees who are not on Elim’s Medica coverage may participate by pre-paying the $50 fee to the Business Office.
Is there a cost to participate in the health screening?
There is no cost to you for participating in the onsite health screening if you are on Elim’s health insurance with Medica.
Will I need to fast before the health screening?
No.
How do I need to prepare for the onsite health screening?
1. Schedule your screening appointment using the online registration tool located at:

2. Drink plenty of water prior to your health screening.
3. Bring your Medica card.
Am I required to schedule an appointment?

to schedule your appointment for the screenings.

• If you are a first time user, enter your email address, create a personal password and click login/create account. Complete all fields with an asterisk to setup an account then click update.
• If you are an existing user, enter your email address and personal password
and click login/create account.
2. Select a time slot from the options available.
3. Select when you would like to be reminded of your appointment and then click Create Appointment. Your appointment is now booked!

Can I cancel my appointment?
Yes, to cancel or change your appointment, go to https://pickatime.com/elimcare.htm. Login and select the event name you originally scheduled an appointment for. You will be directed to the main page. Click on ‘Cancel’ to cancel your existing appointment. You can cancel up to the day of your scheduled appointment.
How accurate is the equipment used during the onsite screening?
The results received during an onsite screening are comparable to a blood draw performed by your doctor. The machines used by US Wellness are calibrated and have quality control checks to ensure accuracy. Please remember that a person’s cholesterol level does change frequently and is affected by the food they eat, any medication (prescription and over the counter), their body chemistry, and metabolism.
I had my cholesterol measured by my doctor recently. Do I still need to participate in the onsite screening?
If you have had these screenings done at your doctor’s office within the last 12 months, you may use the Health Action Notification Form available at MyMedica.com or from your Business Office to submit these results.
What happens to my health screening data after the event?
US Wellness collects the data and stores it in a HIPAA compliant database for 7 years. US Wellness ensures that all participant identifiable information is kept private and confidential and is not distributed to any other organization without your express permission. Who is providing the onsite health screenings?
The health screening provider is US Wellness. (www.uswellness.com) US Wellness is a licensed national health screening and education company that specializes in health and wellness services.
What are the qualifications of the US Wellness health screening personnel?
Each screening technician is a trained health care professional, including registered nurses (RN), licensed practical nurses (LPN), licensed vocational nurses (LVN), respiratory therapists, and medical technicians.
