For individuals whose health and physical needs can no longer be met at home, Elim Care & Rehab Center offers excellent 24-hour skilled nursing home care. Our innovative programs and services focus on the holistic health of our residents and are always administered by highly qualified staff in a compassionate, resident-centered manner.
Rehab Nursing Care
Our therapists and our care-giving team work closely together to develop and execute individualized, incremental rehabilitation care plans that encourage residents to get stronger quickly.
Through these individualized efforts, residents often are able to return to their normal routines. Short-term rehabilitation programs are also available and allow individuals to return to their home following surgery or other health concerns.
Our specialized rehabilitation household called “Turnaround Point” offers residents physical, occupational and speech rehabilitation therapies five to six days a week. Through these services, residents work to regain confidence in their physical strength, fine motor functions and speech abilities.
Memory Care (Alzheimer’s)
Within the skilled nursing facility, residents facing the trials of Alzheimer’s disease, dementia and related disorders can be actively attended to in our safe and secure 17 bed memory care household called “Rum River Place.” The compassionate staff members on Rum River Place are specially trained to care for those with memory loss and adapt activities to meet each individual’s capabilities while still providing them with meaningful social interactions.
Designed for the caregiver who needs an extended break, families may bring their loved one to Elim Care & Rehab Center for a stay that may range from a week to 30 days. Whether they stay with us for only a short time or for a longer time, be assured your loved one will be part of our family.
Home Care Services

In order to provide for the needs of our terminally ill residents and their families, Elim Care & Rehab Center offers hospice services. Through this program, residents and families can receive the additional support, care and comfort they need while working directly with staff who are trained in pain management and comfort care needs. Adult Day Care

Full-time and part-time chaplains are available to meet the spiritual needs of our residents, as each chaplain is trained to serve in a long-term care setting. Spiritual leaders from the community are also invited to speak at our Sunday and Wednesday chapel services, as well as special services throughout the month. Each household has a weekly worship service or Bible study during which elders are encouraged to participate and use their gifts to help lead as they are able.
